I think this is essentially the structure we'll go with, and I think it
has every chapter we need.

Things that have changed:

* significant changes to the first 1/3rd, much of it re-written
* Denis' outline of the WS chapter is in
* all edits provided by reviewers are in (with the exception of the
parameter manipulation chapter which I'm thinking needs a good solid
re-write). Thank you to the reviewers!
* authorization chapter has been fleshed out and authorization items from
authentication moved to the right spot.

I still have some new content to go in from last week.

Christine, I'm sorry I don't have your name to add to the frontispiece
where I am at the moment. The next draft will contain your name. Thank you
(and Frank Lemmon) for your extensive edits.
