[Esapi-php] Codec errors

Just committed a fix in Codec and CSSCodec that fixes the failing
CSSCodecTest as well as fixing Issue 27 (which I've updated to 'fixed' [1]).
So it's only the HTMLEntityCodecTests to address now (just in case,
Mike, you were expecting to see no failing Codec tests in the morning ;) ).
I look at those tomorrow.
