Finally committed all the stuff I've been meaning to commit for too long now! I.e. refactored utility methods between Codec and its extensions, decode method in Codec, and decodeCharacter structure for Codec extensions.

@ Mike: I essentially overwrote your multibyte implementations of encodeCharacter method for PercentCodec, UnixCodec, and WindowsCodec since refactoring between Codec and its extensions affected each implementation. You had done it perfectly though, top job ;) Should add your name to the @author for those files (I wasn't sure how to format that at the time). Would be great if you could scrutinize the decode related methods at this stage.

Updated Issues 23, 24, and 25 accordingly.

Updated @since and (C) notices for Codec related classes.

Following encoder related issues/ideas in main ESAPI list.

@all:  y'all should not be seeing any errors currently stemming from HTMLEntityCodec, PercentCodec, UnixCodec, and WindowsCodec. Please let me know if any pop up!
